Our New Patient Forms are now available online so that you can complete them at your convenience, giving you time to carefully review our policies and procedures without having to do so at our office. Click on the link below to fill out the form and submit it directly to the doctor. It's easy--no downloading or printing.
Our online patient portal is a convenient, secure way to view your past visit history, invoices, and get message from our office. Simply click the button below to create an account or login using your unique username and password.

We are committed to providing dental care that is both high quality and affordable. Payment options include cash, credit card (see list), discounts, checks, major insurance. We also accept major PPO insurance plans (in network pricing).
If you would like to extend payments over a longer period of time, we offer CareCredit.
